Skilled Home Health Services

Our Skilled Nursing services are delivered by licensed Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) in the comfort of your home. These services include comprehensive assessments, medication management, chronic disease monitoring (such as heart failure, COPD, and diabetes), post-surgical care, wound management, and coordination with your physicians to ensure optimal health outcomes. Our skilled nurses are compassionate professionals committed to restoring your health and supporting your independence.

Wound Care

Our expert nurses deliver specialized Wound Care services for acute, chronic, or surgical wounds. This includes assessment, cleaning, dressing changes, infection prevention, and use of advanced wound care products. We focus on promoting healing, preventing complications, and educating patients and caregivers on proper wound management at home.

Infusion Therapy

Infusion Therapy enables patients to receive intravenous medications such as antibiotics, hydration, pain management, or nutrition (TPN) at home. Our skilled nurses coordinate with physicians and pharmacists to administer therapy safely and monitor for reactions or complications. This service minimizes hospital visits and supports patient comfort and recovery at home.

Diabetic Teaching

Our Diabetic Teaching program empowers patients with the knowledge and tools to manage their diabetes effectively. Nurses provide one-on-one instruction on blood sugar monitoring, insulin administration, medication adherence, dietary planning, and lifestyle changes. Education is personalized to improve self-care, prevent complications, and promote long-term health.

Physical Therapy

Our licensed Physical Therapists help patients regain strength, improve mobility, and restore physical function after injury, surgery, or illness. We create personalized rehabilitation plans that may include therapeutic exercises, balance training, pain management techniques, and mobility aid instruction. Our goal is to help you return to your daily activities safely and confidently.

Speech Therapy

Speech Therapy services are provided by certified Speech-Language Pathologists (SLPs) to address communication, speech, language, and swallowing disorders. We help patients recovering from strokes, neurological conditions, or injuries regain their ability to speak, understand, and safely swallow. Therapy is tailored to each individual’s needs for improved communication and quality of life.

Fall Prevention

Fall Prevention services involve a comprehensive assessment of your home and personal risk factors to reduce the chance of falls. Our team provides strength and balance exercises, safety recommendations, assistive device training, and education for caregivers. Preventing falls is key to maintaining independence and avoiding injury.

Gait Training / Strengthening

Our Gait Training and Strengthening program is designed to improve mobility, balance, and lower body strength, often after surgery, stroke, or a prolonged hospital stay. Physical Therapists guide patients through targeted exercises and walking techniques to help restore a safe and efficient gait pattern, enhancing confidence and independence in daily movement.

Home Health Aide

Our Home Health Aides assist with essential personal care tasks under the supervision of a nurse or therapist. These include bathing, dressing, grooming, toileting, mobility assistance, and light housekeeping. Home Health Aides are trained to observe and report changes in your condition, contributing to a safer and more comfortable home environment while supporting your daily independence.

Respite Care

Our Respite Care service provides temporary relief to primary caregivers by offering professional, short-term care for their loved ones. Whether you need a few hours or several days of coverage, our trained staff ensures the continuity of quality care so caregivers can rest, attend appointments, or manage other responsibilities with peace of mind
