A Personalized First Visit
Your first appointment is focused on understanding your unique needs and recovery goals. Our physical therapist will review your medical history, discuss your symptoms, and perform a comprehensive evaluation to identify the root cause of your condition.
Based on your assessment, we’ll create a personalized treatment plan and explain each step of your recovery. You’ll leave with a clear understanding of your condition, treatment recommendations, and, when appropriate, a home exercise program to support your progress.
What Required
To help us provide the best possible care and make your visit as smooth as possible, please bring the following items:
- Completed New Patient Forms to reduce wait time.
- Photo ID and Insurance Card for registration and verification.
- Physician Referral (if required by your insurance plan).
- List of Current Medications and any relevant medical records or imaging reports.
- Comfortable Clothing that allows easy movement during your evaluation and treatment.
We accept most major insurance plans and work closely with providers to make your treatment process smooth and hassle-free. Please bring your insurance card and related details so we can quickly verify your coverage and benefits before starting your care.
